At Healthy Minds Rx, we believe everyone deserves access to quality mental health care—regardless of income. Our Sliding Fee Discount Program helps reduce the cost of services based on your household income and family size.
What is a Sliding Scale Discount?
A sliding scale discount means the cost of care is based on how much money you make and how many people are in your household. If you earn less, you pay less. If you earn more, you pay closer to the regular price. To qualify, you’ll need to show proof of income. This helps make sure everyone can get the care they need, no matter their financial situation.
Who Is Eligible?
You may be eligible for a discounted rate if your income falls within certain limits. We determine your discount based on income brackets aligned with the Federal Poverty Guidelines (FPG). To apply, simply provide proof of income or household size, such as:

Driver’s license or other state-issued ID

Recent pay stubs or Latest tax returns

School ID's or Recent proof of custody (if applicable)

Government benefits letter (e.g., for assistance programs)

Profit-and-loss statement or bank statements (if self-employed)
Note: Discounts may vary depending on the service. Prescription medications and third-party lab tests are not included.
Annual Review & Income Updates
Eligibility is reassessed annually or sooner if your financial situation changes.
Patients must submit updated income documentation each year to continue receiving the discount.
